Submit This Form Online Just provide your credit card information and list which Guests are authorized to charge items to your account. Then get ready to enjoy a carefree cruise.
This form must be submitted, along with the Guest Information Form, before you can access the Disney Cruise Line Cruise Contract and, subsequently, the Online Check-In Port Arrival Form.
If you do not submit these forms online at least 48 hours prior to boarding, you will need to complete them—and the forms you would access subsequently—at the port terminal.
